Medicare Covers In-Home PT, OT & Speech — No Homebound Required

One of the most common misconceptions we hear is: “I thought Medicare only covers home PT if I’m homebound.” That applies to the Medicare home health benefit (Part A) — but Holsman’s in-home therapy is billed under Medicare Part B outpatient therapy, which has no homebound requirement whatsoever.

If you have Medicare Part B, you can receive in-home physical therapy, in-home occupational therapy, and in-home speech therapy services simply because you prefer the convenience — no physician order required to start in NJ, and no need to prove you’re confined to your home.

Standard Medicare Part B cost-sharing applies: 80% covered after the annual deductible, with 20% patient responsibility (often covered by a Medigap supplement).

No. Under Medicare Part B, you do not need to be homebound to receive in-home physical therapy services. Homebound status is a requirement for the Medicare Part A home health benefit — but Holsman’s in-home PT, OT, and speech therapy are billed as Medicare Part B outpatient therapy, which carries no homebound requirement. You can still drive, run errands, and leave your home freely while receiving in-home therapy.

No. New Jersey’s Direct Access law permits patients to begin physical therapy — including in-home PT — without a physician referral for up to 30 days (or 12 visits). After that, a referral is required to continue. Most insurance plans including Medicare Part B cover Direct Access visits. We verify your coverage at no charge before your first visit.
