A 7-question survey that turns how your knee feels into a 0–100 score you can track over time.
The KOOS, JR. (Knee injury and Osteoarthritis Outcome Score for Joint Replacement) was developed at Hospital for Special Surgery in New York. It asks 7 questions about your knee over the last week and gives a score from 0 to 100, where 100 means perfect knee health. Doctors and physical therapists use it to track knee arthritis and recovery before and after knee replacement. It takes about 2 minutes. Use it to see your starting point and how you change with treatment.
Instructions
This survey asks for your view about your knee. This information will help us keep track of how you feel about your knee and how well you are able to do your usual activities. Answer every question by ticking the appropriate box, only one box for each question. If you are unsure about how to answer a question, please give the best answer you can.
On this page, tap one answer for each question instead of ticking a box.
The following question concerns the amount of joint stiffness you have experienced during the last week in your knee. Stiffness is a sensation of restriction or slowness in the ease of which you move your knee joint.
What amount of knee pain have you experienced in the last week during the following activities?
The following questions concern your physical function. By this we mean your ability to move around and to look after yourself. For each of the following activities please indicate the degree of difficulty you have experienced in the last week due to your knee.
This survey tracks symptoms and function. It is not a diagnosis. It shows how your knee feels and works from your point of view. It can’t tell you what is causing your knee problem or what treatment you need. Only a licensed health care provider who examines you can do that. If you have severe or sudden knee pain, swelling, redness, fever, or you can’t put weight on your leg, call your doctor right away.
Each answer is scored from 0 (None) to 4 (Extreme). The 7 answers are added up to a raw score from 0 to 28. The raw score is then converted to a score from 0 to 100 using the official table from Hospital for Special Surgery, where 0 represents total knee disability and 100 represents perfect knee health. HSS does not publish “normal” or “abnormal” ranges for individuals, so the most useful thing is to compare your own scores over time.

The Knee injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS, JR.) is a 7-question survey developed at Hospital for Special Surgery (HSS). It asks about knee pain, stiffness and daily function over the last week. It is widely used to track knee arthritis and recovery before and after knee replacement.
Each answer is scored 0 (None) to 4 (Extreme). The answers are added up to a raw score from 0 to 28, which is then converted to a score from 0 to 100 using the official HSS conversion table. 100 means perfect knee health and 0 means total knee disability.
Higher is better, and 100 is the best possible score. HSS does not publish good or bad ranges for individuals. The score is most useful for tracking your own knee over time, for example before and after physical therapy or surgery.
No. It measures how your knee feels and works from your point of view. It does not diagnose arthritis or any other condition. Only a licensed health care provider who examines you can do that.
Use it whenever you want a snapshot, such as before starting physical therapy and every few weeks during care, or at the times your surgeon or therapist asks. Print or save each result with the date so you can compare.
